Application Process

Your application packet should include one copy of the following:

  • Letter of Application -
    Include in your letter what attracted you to the internship at Illinois State University. In addition, briefly describe individual traits and how they might compliment this internship. Lastly, characterize a situation where you experienced a conflict with a fellow student/employee/other and how you handled it.
     

  • American Dietetics Association Dietetic Internship Application Form (including GRE scores). Click here to access the NEWLY revised Supervised Practice/Dietetic Internship Application
     

  • Three letters of recommendation - as a part of the DI Application - from individuals who have knowledge of your ability to succeed.  One recommendation must be written by a faculty member at the institution where you completed your DPD program and one should be from an employer/supervise. ADA  DI Recommendation Form
     
  • Current resume
     
  • Original transcripts - from all colleges and/or universities attended
     

  • Verification of Completion of Program or Declaration of Intent to Complete Degree form
     

  • The Graduate Assistantship Application - that was stated earlier is no longer needed with this packet. Those applicants who are matched to the program will complete an online form in April. Please contact Julie Raeder Schumacher if you have any questions.

Computer Matching:

All applicants to the Dietetic Internship at Illinois State University must participate in computer matching. Applicants should request instructions and a mark/sense card to prioritize their DI selections. Applicants should request this material from any ADA Approved/Accredited Didactic Program in Dietetics, or from D&D Digital Systems.

This request should be made to allow sufficient turn around time for submitting materials by the February postmark deadline. There is no charge for this material. However, there is a $50 charge for computer matching that is due with the applicant's prioritized ranking.
